General Practice Data for Planning & Research (GPDPR)

From 1st July 2021, NHS Digital will collect data to support a wide variety of research and analysis to help run health and care services.  In additions, the service will also help to support the planning and commissioning of health and care services, the development of health and care policy, public health monitoring and interventions and enable many different areas of research.

New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.  These can either be brought into the surgery or emailed to the practice email address: gmicb-bol.p82037@nhs.net   Please ensure that you provide a contact telephone number and the forms are completed in full.

If you are within our practice area you will be offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

In line with the new GMS contract, all patients now have an allocated GP.  This may not be the GP that you usually see and will not affect your right to see whichever GP you choose.  In most cases, your allocated GP will be the GP who you are registered with.  If you are not sure and would like to know, then please ask at reception.
